Medical Homebound
Homebound services provide students unable to attend in-person school due to a medical or mental health condition with educational instruction. The Homebound form must be completed by a licensed physician detailing the diagnosis, the inability to attend, and the expected duration. This form must then be returned to the PowerSchool secretary at the school. The homebound team will then review the application and determine approval. If approved, the school is responsible for ensuring academic continuity. Continuation of homebound services should be reviewed throughout the school year and homebound services do not transfer over from school year to school year. A new form is required every school year.
